Application Forms

Application forms for performers, song contest entrants, artists & craftsfolks, and food vendors may be downloaded from the following links. Forms are available in Portable Document (pdf) format. If you are unable to download and open a pdf file, please call or e-mail 3RFS and we will mail you a printed copy of any of these forms.

Printed copies of applications, along with any promo materials and music samples, should be mailed to 3RFS. Sorry, but 3RFS does not have sufficient staff or hardware to support electronic submittals. Please note that the submission deadline is earlier this year.

Performers

The performer material includes a Dear Performer letter, a Performer Policy, an Application Form, and a Tech Sheet. Performer applications should be printed, filled out and sent by mail, and must be received by May 10, 2008.

Songwriters

The song writing contest material includes a Dear Song Contestant letter, a Song Contestant Entry Form, and a Tech Sheet. The theme for 2008 is Generations. Song writing contest applications should be printed, filled out, and sent by mail, and must be received by May 10, 2008.

Artists and Craftsfolks

The artist/craftsfolks material includes a Dear Artist letter, an Artists Prospectus, and an Application Form. Applications should be printed, filled out, and sent by mail, and should be received by July 1, 2008. Applications received after this date will be considered on a space-available basis and, if accepted, will be assessed a late fee.

Food Vendors

The food vendor material includes a Dear Concessionaire letter, a Consessionaires Policy, and an Application Form. Applications should be printed, filled out, and sent by mail, and should be received by July 1, 2008.
